A blank is not a pass. A day with no reading counts as no data, never as on target. Hover or focus a cell on a screen with a pointer to see its value.
Radar
By heading
By measure
The score is the share of working days in the period on which a measure was on target. A working day with no reading counts against the score, so a board that is not filled in cannot score well. The period ends on the board date, and the dashed line is the period of the same length before it. The score says nothing about whether a reading is true.
Measures
Set a target and a red limit for each. For a measure where lower is better, on target is at or below the target and red is above the red limit. For a measure where higher is better, on target is at or above the target and red is below the red limit. Between the two is amber. With no red limit there is no amber band.
Working days
Days with no reading on a working day are counted as missing. Pick the days your shift or site actually runs.
What the checks found
The checks look for gaps in the record: missing readings, misses with no action, actions with no owner or date, escalations not recorded, and actions not independently checked. They cannot tell whether a reading is true or whether an action will work.
What it is for
A tier meeting is a short, standing meeting held at the same place and time each day, working up from the team to the area to the site. Each tier looks at the same five headings, Safety, Quality, Delivery, Cost and People, and deals with what it can. What it cannot fix goes up to the next tier. This tool holds the numbers, the reasons and the actions for one board. It does not run the meeting.
How a reading is judged
- Green is at or better than target. Amber is worse than target but not past the red limit. Red is past the red limit. With no red limit, a reading is either green or red.
- A blank reading is "no data". It is never shown as green.
- A measure with no target cannot be judged, and the checks say so.
- Colour is never the only signal. Each status also has a letter.
What the checks look for
- A measure with no target, or a red limit on the wrong side of the target.
- Missing readings on working days in the last five.
- A measure that is red now with no open action against it.
- A measure red on three readings in a row with nothing escalated.
- An action with no wording, owner or due date, or one that is overdue.
- An action marked done with nobody named as having checked it, or checked by its own owner.
- An action marked for the next tier with no date recorded for when it was raised there.
The scorecard and radar
The Scorecard tab turns the readings into one number for each heading: the share of working days in the period on which the measures were on target. A working day with no reading counts against the score, so an empty board cannot look good. The radar draws the five headings on one chart, with the period before as a dashed line. It needs measures under at least three headings. Use it to see where the board is weakest, not to compare people.
Reading the board
A board that is mostly green may mean the targets are too easy or the readings are not being taken honestly. Look at trends, ask the people who do the work, and keep the red limit where it would really trigger a response. Readings you type in are not checked against a source.

No gaps found does not mean the board is right or the process is under control. Check your own company procedures. This is decision support only.
